Special Needs Plans (SNPs – pronounced snips) are plans that provide benefits and services to people with specific diseases, conditions or limited income. These plans must cover all the basic benefits of Medicare Advantage plans, but they also cover extra services tailored to a special group. Plans are designed to provide targeted care. There is limited enrollment and you must qualify for a SNP plan. We can help confirm your eligibility, call us at 866-MEDIGAP or 866-633-4427. Our Services are absolutely free to you!
Each plan must have a Model of Care which is important to improve patient outcomes. These plans traditionally offer better benefits and services for the people they service. There are three categories of special needs patients:
- Dual Eligible
- Chronic Health Conditions
- Institutional
